Early relational and complex trauma usually involves repeated experiences involving fear of a caregiver. While a caregiver may frighten their infant with angry aggressive behaviour, fear can also be instilled by non-abusive behaviours that are involuntary or unconscious and which may even be the product of illness, loss or separation. These repeated frightened or “dissociative” behaviours result in confusing, disorientating and disorganising effects through infancy and childhood, and are a risk factor for dissociation in adulthood.

When working in an infant mental health space, it is important to understand that caregivers have often experienced their own early relational trauma and later trauma and loss. The caregiver's early relational trauma plays out in the dyadic relationship. This explains the urgency of providing a trauma-informed dyadic therapeutic intervention.
